Hospital-to-home checklist for an aging parent

Before discharge: ask for a written handover

Ask the care team to explain the plan to the person who will help at home. Tell them if the support they expect will not be available.

• Who has the discharge summary, current prescriptions and follow-up instructions?
• What help or equipment is needed, and who explains how to use it?
• Which appointments are booked and which still need arranging?
• Who should we contact with questions, including after hours? Ask the team which warning signs require urgent help.

First 24 hours: cover the practical essentials

Agree transport, home access and who will be there. Keep walking routes clear and put the phone and everyday essentials within reach.

• Keep written instructions and care-team contacts together.
• Follow the discharge prescriptions only. Ask a clinician or pharmacist to clarify differences from the old list.
• Check access to the prescribed medicines and recommended equipment.
• Name the person responsible for meals, transport and calls.

Hours 48–72: check the handover is working

This is an organisational review, not a clinical timetable. The care team’s instructions take priority.

• Confirm follow-up appointments and transport.
• Check that agreed home support has a contact and start date.
• Review family cover and identify gaps.
• Write down questions and observations with dates and times; leave clinical interpretation to professionals.

Make the next caregiver’s shift easier

Keep one dated version of the essential information. Show the next caregiver where it is and what still needs doing. Ask your parent what they want shared and with whom.
